A Vestal manufacturer was hit with several violations following a deadly forklift accident earlier this year. OSHA wrapped up its investigation into the March 22nd incident at National Pipe and Plastics. A truck driver from another company was hit and killed by a forklift driven by a National Pipe and Plastics employee. The company is being cited for 16 serious health and safety violations, two directly related to the accident. Investigators say the forklift’s backup alarm wasn’t working and the driver didn’t have a clear view of the path. OSHA says inspectors also found combustible dust on surfaces, improper stairs and handrails and other violations. OSHA is proposing penalties totaling $67,000. National Pipe and Plastics has 15 business days to comply with or contest the fine.
